Course Interruption / Cancellation Program

 

Introduction

 

The Superior EMS course interruption / Cancellation program covers all the expenses related to transferring to a new course date or the requirement to cancel a course registration completely due to several situations that may arise. 

 

This program is for protection arising from sudden and unforeseeable circumstances, and a claim with supporting documentation is required to be covered under this voluntary program.

 

Definitions

 

Participant means the individual who is the registered individual for the training course.

 

Covered Person is the participant.

 

Course Date is the date in which the scheduled course starts.

 

Dependent Child means an unmarried, natural, adopted, step or foster child, or legal ward of the participant who is:

·      Under 21 years old

·      Mentally or physically handicapped and incapable of self-sustaining employment and totally reliant on you for support and maintenance.

 

Effective date means the date and time of purchase of the prepaid training course, where you have paid the entire cost of the course.

 

Emergency means any unexpected or unforeseen sickness or bodily injury that occurs during the period of converge and makes It necessary for you to receive immediate treatment from a physician or nurse practitioner, or to be hospitalized.

 

Family means your spouse, parent, step-parent, grandparent, grandchild, in-law, natural or adopted child, step-child, brother, sister, step-brother step-sister, legal guardian, or legal ward.

 

Hospital means an establishment that is licensed as an accredited hospital, is operated for the care and treatment of in-patients, has a registered nurse always on duty, and has a laboratory and an operating room on the premises or in facilities controlled by the establishment. Hospital does not mean any establishment used mainly as a clinic, extended or palliative care facility, rehabilitation facility, convalescent, rest or nursing home, home for the aged, health spa or addiction treatment centre.

 

Key employee means an employee whose continued presence is critical to the ongoing affairs of the business during your absence.

 

Medical condition means accidental bodily injury or sickness (or a condition related to that accidental bodily injury or sickness), including disease, acute psychoses and complications of pregnancy occurring within the first thirty-one (31) weeks of pregnancy.

 

Mental or emotional disorders means emotional or anxiety states, situational crisis, anxiety or panic attacks, or other mental health disorders treated with minor tranquilizers or anti-anxiety (anxiolytics) medication.

 

Physician or Nurse Practitioner means someone who is not you or a member of your family who is licensed to prescribe drugs and administer medical treatment (within the scope of such license) at the location where the treatment is provided. A physician does not include a naturopath, herbalist, homeopath or chiropractor.

 

Spouse means the person who is legally married to you, or has been living in a conjugal relationship with you and residing in the same household as you for a continuous period of at least one (1) year

 

When Does Coverage Begin and End?

 

This program provides coverage whenever a prepaid course is made with Superior EMS, and the appropriate fee is paid for this voluntary coverage. 

 

Coverage starts on the effective date.

 

Coverage ends Midnight the day before the scheduled course

5.     A transfer by the employer with whom you or your spouse is employed on your effective date, which requires the relocation of your principal residence.

No Questions Asked Cancellation

 

If you need to cancel your registration for any reason you are covered under the following terms:

 

Up to 30 days before the course

90% Refund

20-29 days before the course

80% Refund

14-19 days before the course

70% Refund

5-13 days before the course

50% Refund

 

Refunds will be issued on the original method of payment. 

 

Emergency Cancellation Refund

 

Should Superior EMS cancel your course and reschedule it for another date, and you wish not to complete the course at the new date offered, or transfer to another date offered, you are eligible for a 90% refund of the course fee.   

 

General Exclusions

 

1.     Your failure to show up for the scheduled course, except in circumstances described as covered reasons

2.     Any medical condition if you undertake the course with the prior knowledge that you will require or seek treatment, surgery, investigations, palliative care or alternative therapy of any kind, regardless of whether the treatment, surgery, investigations, palliative care or alternative therapy is related in any way to the medical condition.

3.     Your intentional self-inflicted injury, suicide or attempted suicide (whether sane or insane)

4.     Your mental or emotional disorders

5.     Any medical condition, arising from, or in any way related to, your chronic use of alcohol or drugs whether prior to the course

6.     Your abuse of medication, drugs or alcohol or deliberate non-compliance with prescribed medical therapy or treatment whether prior to or during your course.

 

What should you do if you wish to make a claim?

 

For your claim to be reviewed, and approved you must submit the following information:

·      The completed claim form online

·      The medical certificate, fully completed by a physician or nurse practitioner.

·      Proof of death

·      Other documents as required

 

For the no-questions asked cancellation refund request, the claim form must be completed online.

 

You must submit the information required for your claim within thirty (30) days of the date of the cause of cancellation or reschedule.  Approval or denial of the claim will be provided in writing within thirty (30) days of receipt of the required documentation. 

 

Approval is at the sole discretion of Superior EMS management. 

 

All course rescheduling must occur within 1 calendar year of the original course, and must be the same course registered for initially.
